Whanau Ora - Better, Sooner, More Convenient Business Case
Description: Building on the practical experience of Whanau Ora (a holistic view of individual/family health and wellbeing) within the National Maori PHO Coalition, Synergia used its expertise in social services, people centric health and integrated models of care to help the coalition prepare their business case for an integrated health and social service approach to address the needs of Maori and other high needs populations.

Better, Sooner, More Convenient Primary Health EOI for Hawkes Bay Public Health Organisation
Description:
Building on prior strategic work by Synergia with the DHB, the Hawkes Bay PHO used the opportunity provided by the government's 'Better, Sooner, More Convenient' initiative to develop an action plan for integrated family health centres as hubs for the integration of community, primary and hospital services.

Development of 5-Year Business Plan
Description: Building upon work we had already done in helping redesign renal services and developing improved services for chronic care management, Synergia was asked to facilitate the development of a 5-year strategy that could be used as the overall framework to integrate diverse activities and guide Hawke's Bay DHB in improving its financial and service performance.

Waitemata Family Health
Description: Under the 'Better, Sooner, More Convenient' initiative, Synergia supported Waipareira Trust, Appollo and Coast to Coast PHO to develop an action plan that integrated their respective strengths in community based care, integrated primary health centres and multi-disciplinary team care into a 'single system of care'; integrated services delivered across home, community and hospital settings, shared information infrastructure to provide responsive and coordinated care linked to a clinical network of multi-disciplinary practitioners.

Better, Sooner, More Convenient Primary Health Care
Description:
Developing a successful proposal representing 3 District Health Boards and 10 Public Health Organisations to improve primary healthcare performance and care integration for more than 1 million people. The proposal included changes to models of care, clinical leadership and suppporting infrastructure and was followed by development of a business case for implementation.

Auckland Regional Health Impact Assessment Strategic Review
Description:
Identifying opportunities for health impact assessment in the Auckland region and charting a forward programme for its development and leadership by the Auckland Regional Public Health Service. The research process included stakeholder interviews, a multi-sectoral workshop, document analysis, and an online survey.

Description:

Nelson Marlborough Primary Care Strategy           
Description:
Working with Nelson Marlborough DHB and the two Primary Health Organisations to develop a long term strategy and operational framework for primary health care.

Pacific Workforce Development Plan                         
Description:
Development of a long term plan for the Pacific health workforce throughout Auckland District Health Board, spanning all arms of health services, and focusing particularly on young Pacific people, nursing and allied health development, and career development and leadership pathways.

Renal Service Design                                                        
Description:
This assignment focused on redesigning the current renal service in response to growing demand for in-centre dialysis services. The work used System Dynamics modelling to explore the drivers of renal demand and the clinical and financial outcomes of different design options.

South Island Population Health Strategy                 
Description:
Development of Healthy South - a strategy for developing population health capability for the whole South Island. Involved detailed analysis and widespread stakeholder engagement. Undertaken for Ministry of Health and all South Island District Health Boards.

Wellington Region Population Health Strategy       
Description:
A five year strategy identifying clear priorities for the three District Health Boards and including evidenced based outcome frameworks to drive collective action to achieve improvements in health status for the population groups most at risk.

Description:
A five year strategic implementation plan for Healthy Eating Healthy Action across three district health board areas, includes aligning the Ministry of Health and the district health boards' needs and aspirations.

Modelling Diabetes in the Pacific Population           
Description:
As part of the National Pacific Diabetes Strategy, this work was undertaken to explore the impact of different strategies upon the prevalence and incidence of obesity and diabetes amongst the Pacific population in Counties Manukau.

Our Health 2020                                                              
Description:
Our Health 2020 is a broad plan and process to focus Auckland DHB on its whole system responsibilities. Developed Health Improvement plans with whole system focus for priority health areas. Our Health 2020 has resulted in more strategic approach to improve performance across the sector.

Green Prescriptions Review                                           
Description:
Analysis of Green Prescription (GRx) and development of future strategy. The work resulted in $10 million investment by Government in expansion of GRx as part of Mission On to move from a boutique programme to core sector infrastructure.

Description:
A whole system strategy to reduce diabetes incidence and improve quality of care. Focus for $10 million investment from Counties Manukau District Health Board. Let's Beat Diabetes is now entering year 3 of implementation. Seen as an exemplar programme of international significance.

Modelling Investment Strategies for Let's Beat Diabetes
Description:
As part of the development of the Let's Beat Diabetes programme, this assignment used System Dynamics modelling to explore the prevalence and incidence of obesity and diabetes for the population of Counties Manukau under different investment strategies.

Framework for Preventing and Managing Chronic Disease 2004-2008
Description:
